APPLICATION TO ADOPT
(This application is for parents wanting to adopt a child. If you are a birth parent considering adoption for your child, please click on the Birth Parent button.)
If you are interested in adopting through our office, please print out and complete our application package. Once the application package is completed, you may either mail or overnight the package to us at the address below. Upon receipt of your package, we will schedule a one-hour consultation with you. The consultation may be done in person or, if you are out of our immediate area, by teleconference. During the consultation, Attorney Kaplan will discuss with you the adoption process, the procedures of our office and Florida adoption law. She will also answer any questions you may have regarding adoption as well as help you create an individualized adoption plan.
