Dear Prospective Adoptive Applicant:
Thank you for visiting our website. We hope the information you obtained from the site was helpful.
Should you wish to proceed with an adoptive placement through our office, please complete the attached Adoptive Home Application, Adoption Disclosure and Risks in Adoption and return to me along with a copy of your marriage certificate, any prior divorce decrees (if applicable) and a check in the amount of $250.00 made payable to The Law Offices of Ellen M. Kaplan, P.A. for your initial interview and consultation. Your initial interview and consultation can be done either in person or over the telephone, whichever you prefer. In addition, we will also need a copy of your adoption home study, if one has already been completed, and your prospective adoptive parents' profile. (A sample profile is attached. The profile should be a single sheet of paper, front and back, and no larger than 8 1/2” X 11”. When selecting pictures for your profile, remember to choose a variety of pictures that reflect who you are and the things you love to do.)
